What is "personally identifiable information?"

Through our Walk Talk Track (WTT) application, we may request information from you including name, email, date of birth, residence, diagnosis with certain disease states, weight, height and whether or not you take medications for your disease states. The information requested of you is study specific, and may not include the aforementioned personally identifiable information.

What security procedures are in place to protect the loss, misuse or alteration of the information?
Access to the Services, including to databases containing personally identifiable information, is protected by industry-standard encryption. Any personal information that you have posted in any portion of the Services is accessible only by the research coordinator associated with your research study.

We use industry-standard software, hardware and other measures, to protect the confidentiality, security and quality of information we collect, and to protect it from unauthorized access or disclosure or accidental loss, alteration or destruction. Due to the nature of the Internet and computer security, we cannot warrant or guarantee that security will never be breached.

How do we protect children's privacy?
You must be 13 years of age or older to register to use the Services. We do not knowingly collect personally identifiable information from children under the age of 13. If such information is inadvertently or unknowingly collected, it is our policy to destroy or delete it upon discovery.

Transfer of Data Internationally.

PHaware is headquartered in the United States and does not have affiliate offices and partners in other countries. Please be aware that information you provide to us or that we obtain as a result of your use of our online Services may be collected in your country and subsequently transferred to the United States. The privacy and data protection laws in the country to which your information is transferred may not be equivalent to such laws in your country of residence. This Online Privacy Policy is governed by the internal substantive laws of the State of Colorado, without regard to its conflict of laws principles. Jurisdiction for any disputes, controversies, and claims arising under, in connection with, or out of this Policy shall lie exclusively with the state and federal courts located in Denver, Colorado. If any provision of this Online Privacy Policy is found to be invalid by a court of competent jurisdiction, the invalidity of such provision shall not affect the validity of the remaining provisions of this Online Privacy Policy, which shall remain in full force and effect.
